Has your Physician recommended Movement Therapy?
Clients often come to Active Spine physical therapy with pain and/or fear. Although we respect these pains and fears, our focus is fixed on what these pains and fears are preventing you from doing. We realize that your symptoms are evidence of less than optimal movement and potentially your body’s attempt to protect itself with compensations.
The Active Spine movement therapy process specializes in an authentic functional assessment that not only addresses your symptoms but works to identify and understand what caused them. We then partner with you via an integrated, hands-on approach to relieve your symptoms, restore your movement and renew your approach to living life. Throughout the rehabilitation process we will consistently develop and prescribe an individually based home-workable exercise program.
In the state of South Dakota a referral is not needed, however, if you have a medical need it is strongly recommended that you communicate this with your physician. We accept direct referrals from medical providers such as doctors, nurses, physician assistants, physical and occupational therapists, chiropractors, massage therapists and athletic trainers. We also have a growing number of clients that have asked us to partner with their personal trainer, strength coach or performance and skill coach. Please tell us in advance if your referral source will accompany you to your appointment.
Prior to your first appointment you will be asked to fill out and submit a health history form. Your initial evaluation and treatment session will last no more than 60 minutes. Subsequent sessions, if needed, will be discussed during the first session
At Active Spine we are currently credentialed with the following insurance health plans: Sanford, Avera, DakotaCare, BlueCross/BlueShields and Medicare. We also have a cash rate to assist you if we are not in network for you or if you have a high deductible. Should you choose cash options, you can receive a super-bill (medical receipt) for possible insurance reimbursement. This depends on your individual insurance plan.
